A
disability/PWD certificate is a must-have document for every person with a disability
to avail the benefits of the government schemes that are specially created for
the PWD candidates. the Department of Empowerment of Persons with
Disabilities introduced a new provision of a UDID card(unique disability id
card) to give the facility to carry only one document for the disabled person.
Documents required for UDID card
To apply for a disability/PWD certificate following documents are required.
- Address proof (Aadhaar card, ration card, residence certificate, driving license, voter id card, etc.) You can provide any one of these documents.
- Identity proof (Aadhaar
card, ration card, pan card, driving license, voter id card, etc.) You can
provide any one of these documents.
- Income proof (Income certificate/ An income proof verified by Revenue Inspector)
- Recent pass photo
- Signature/thumb impression
- Mobile number
- Caste certificate(optional)
Online Application:
- To
apply for a disability certificate open any browser on your mobile or computer
then in the search bar search for swabalambancard.gov.in or you can also click
here → official website.
- After that, an official website of a unique disability id will be shown on the screen. Then on the right-hand side, you will see an option Apply for disability certificate & UDID card simply click on that option.
- In the next screen, a form will open and there are 4 categories as Personal details, Disability details, Employment details, and identity details. Now you have to fill in all the details step by step.
- Before
proceeding first you have to choose your regional language by clicking the tab
which is at the top of the website.
Step-1(Personal details)
- In this step you have to enter your First Name, Middle Name and surname now enter the above details in the regional language also. For typing in regional language simply type in the English alphabet and click on the space button once it will automatically convert to your regional language.
- In the next line enter your father's name, mother's name, and your date of birth. Your age will be calculated automatically as per your date of birth.
- Now choose your gender, enter your mobile number & email id. If you have any mark of identification then you can enter that otherwise you can leave it blank as it is not a mandatory field. Then Choose your category, blood group, and marital status. If you are married then enter your spouse's name.
- Now in the next line enter the relation with the disabled person, guardian name, and the contact number of the guardian/caretaker.
- Next, you have to upload a photo and signature/thumb impression. Remember the size of the photo should be between 15kb to 30kb and the size of the signature must be between 3kb to 30kb.
Address for correspondence:-
- In the address, line-1 enter your village name, enter your post office name in address line-2, then select your state, district, city/tahsil, and village/block from the dropdown menu. Then enter your pin code.
- Now select the document which you want to upload for the proof of address and upload the document by clicking choose file. Aadhaar card is highly recommended.
Note:- the file size is between 10kb
to 100kb and it should be in the format of jpg, jpeg, png, and pdf
Permanent Address:-
- If
your permanent address is the same as correspondence then simply click on same
as above. It will fill automatically.
- If both addresses are different then you can enter them manually.
Educational details:-
- Now
choose your educational qualification from the below dropdown menu.
- Click
on Next.
Step-2(Disability Details)
- In this step, you have to choose whether you have a disability certificate before or not. If you click yes then give all the detail of the certificate and if you applying for the first time then simply click on No.
- Now select your disability type from the below box.
- If you have a disability by birth click disability by birth. If the disability is after birth due to some region then click disability since and select your year of disability.
- If your treatment is undergoing you can mention the name of the hospital here. If you have a pension card then enter the pension card number otherwise you can leave it blank.
- Now
select disability due to and the name of the hospital.
- Click
on Next
Step-3(Employment Details)
- In
this section select whether you are employed or unemployed and occupation.
- Enter
your personal and spouse's annual income.
- This
section is not mandatory you can skip this section.
Step-4(Identity Details)
- In
this section choose the document you want to upload as identity proof.
- If
your identity proof document is the same as the address of the correspondent
then click on the box otherwise you have to upload the document.
- Now
enter your Aadhaar number and click on I agree.
- Now
fill the captcha code is shown in the figure on the right-hand side and click
the term and condition box below.
- Click
on proceed.
- A
confirmation window will appear. check your details and click on confirm
application button. If you find anything wrong with the details then click on
edit application and correct it.
- A
new window will appear to verify some basic details simply click on yes
for every field.
- Now your application is complete. A reference number will be displayed writedown the reference for future use.
